Every startup or organization starts off inherently agile, but over time, processes shift from simple to complex, to manage the chaos, abiding by the Second Law of Thermodynamics. As processes gather unnecessary waste in the guise of decision points and stage gates, the resources involved in the processes (with the overall intention of doing good) are conditioned to think of issues or risks that could impact quality of the output, and before they know it, become engaged in simple sabotage, impacting productivity or throughput.
